Position: Community Mental Health Practitioner
Specialisation: Older persons CMHT

Shift Pattern: Monday - Friday | 9am - 5pm
Pay Rate: Day £27ph

Responsibilities:

• Conduct comprehensive mental health assessments for older adults, considering cognitive, emotional, social, and physical health factors.


• Develop individualised care plans to address mental health needs, working in collaboration with patients, families, and other professionals.


• Participate in MDT meetings to review care plans and discuss patient progress.


• Respond to mental health crises, providing urgent support to older adults experiencing acute distress or deterioration in mental health.


• Support patients with dementia and their families in managing symptoms and planning for future care needs.

Qualifications and Requirements:

• Hold a degree in mental health nursing.
  
  


• Fully enhanced valid DBS


• Community experience is essential.

Rate Breakdown
£27ph LTD | £24.29 PAYE Inclusive | £21.68 Exclusive

We offer a £250 bonus for starting a contract with SCS.
