Main Role Responsibilities

• The position is within a hospital discharge team, working alongside health professionals within a multi-disciplinary team.
  
  You will be expected onsite 5 days per week.

• Significant experience of applying appropriate legislation to the area of work for example Community Care Act, Mental Health Act, Mental Capacity Act/D.O.L.S Continuing Health Care.

• As part of the operating model for adult social care in Rotherham, this role will be required to implement a strengths-based approach to delivering Care Act outcomes, ensure that practice includes developing preventative approaches to informal and formal support and the commissioning of services.

• In this role you must be able to work effectively as part of a team and along with a range of agencies and professionals, exercising professional judgement and using appropriate methods and interventions, values, and knowledge base to work with adults and their carers to achieve the best possible outcomes.

Qualifications and Skills:

• Qualified Social Worker with relevant degree or equivalent qualification


• Registered with Social Work England


• Experience working with vulnerable adults


• Ability to work independently and within a team


• Excellent communication and interpersonal skills


• Full enhanced DBS
