Rate of pay - £27.96ltd per hour

The London Borough of Hammersmith and Fulham are seeking a highly motivated and experienced Team Leader to join their team.

The successful candidate will be responsible for providing excellent support to vulnerable adults with a range of disabilities, including physical disability, autism, challenging behaviour, mental health, and dementia.

• Deliver excellent support to vulnerable adults with a range of disabilities


• Work in partnership with schools, colleges, and other service providers to deliver transition groups for residents


• Assist residents in identifying progression opportunities and provide ongoing transition support


• Establish positive relationships with residents and support them to increase their confidence and realise their potential


• Explore services such as independent living establishments and a curriculum for independent living skills

• Proven experience within a supervisor position or in a similar role


• Excellent communication and interpersonal skills


• Ability to work independently and as part of a team


• Strong organisational and time-management skills


• Experience working with vulnerable adults with disabilities
