Outreach Family Support Worker
Are you passionate about making a difference in the lives of children, young people, and families? Service Care Solutions are seeking a dedicated Outreach Family Support Worker to provide high-quality, home-based early help support using a 'whole family' approach.
You will play a crucial role in preventing the need for statutory services while ensuring families have access to universal support.
- £17.00 LTD per hour inclusive of holiday pay
- 35 hours per week
- 2 month initial contract with possibility of extension after this
Responsibilities
Hold and manage complex cases stepping down from children's social care (Level 2/3), completing early help assessments and coordinating Team Around the Family (TAF) meetings.
Provide intensive support to children, young people, and families, including adolescents with complex emotional and mental health challenges.
Assess risk and need using early help assessments, taking on the Lead Professional role where required.
Develop and coordinate family support plans to reduce risk and improve outcomes.
Support families in developing parenting strategies, promoting positive relationships.
Maintain accurate records using the electronic case management system.
Conduct home visits and provide outreach support.
Deliver supervised contact sessions for families under Early Help intervention.
Promote safeguarding in accordance with statutory guidance and legislation.
Collaborate with multi-agency teams, including schools, social workers, and healthcare professionals.
Requirements
Essential:
NVQ Level 3 in Childcare, Education, or Health & Social Care.
Experience of direct work with children, young people, and families.
Strong skills in case management, assessment, and delivering interventions.
Effective communication skills, both written and verbal.
Experience in providing home-based and outreach support.
Understanding of safeguarding and child protection procedures.
Ability to work independently and within a team.
Desirable:
Experience in parenting support and family mediation.
Knowledge of child development and emotional well-being.
Experience of working with families under SEND guidelines (ages 19-25).
Strong IT skills and the ability to maintain accurate case records.
