Responsibilities:

• Act as a link between primary care, secondary care, mental health services, social care, and voluntary organisations.


• Monitor patient progress and arrange necessary follow-ups or reviews.


• Develop tailored care plans in collaboration with patients, their families, and healthcare professionals to meet individual health and social care needs.


• Identify patients at risk of deteriorating health using risk stratification tools and ensure they receive targeted, proactive care.

Qualifications and Requirements:

• Hold a degree in mental health nursing.
  
  


• Fully enhanced valid DBS


• Community experience is essential

Rate Breakdown
£27ph LTD | £24.29 PAYE Inclusive | £21.68 Exclusive

We offer a £250 bonus for starting a contract with SCS.
