Responsibilities:

• Ensure accurate and timely recording of data and updating of care plans using SystmOne


• To be responsible for the assessment, planning, implementation, and evaluation of evidence based individualised programmes of care.


• The post holder will also co-ordinate, manage and organise the nursing caseload.


• To recognise changes in patients' conditions which require the intervention of others and refer on as appropriate.

Qualifications and Requirements:

• Fully enhanced valid DBS


• Must have community experience

£29ph | £23.24 PAYE Exclusive

We offer a £250 bonus for starting a contract with SCS.
