Care Co-ordinator

Service care Solution are currently recruiting for a Care Co-ordinator in Enfield.

As a Care Co-ordinator you will work face to face work with service users, their families, and external partners.

The purpose of the Care Coordinator role is to identify whether support packages continue to meet the needs of service users and their families.

Main Responsibilities

As a Care Co-ordinator, you will be responsible for:

• To undertake initial assessments within the area of specialism.


• To manage, monitor and review care plans involving users and carers and liaising with other workers and agencies as appropriate.


• To be aware of the resources available within the specialist area, including other sources of funding so that the maximum benefit may be obtained for service users within budgetary resources of the Group.


• To make use of the management information systems, including information technology, in the recording retrieval and analysis of information as required by the department.

Requirements:

• Enhanced DBS


• Have experience of working within an integrated team


• Have experience of working with people who have a learning disability

This role is paying £16.30 limited per hour (umbrella rate)
