Community Care Officer

Service Care Solutions have a new and exciting opportunity which has become available working on behalf of Nottingham City Council as a Community Care Officer.

Role Overview:


We are looking for a dedicated and empathetic Community Care Officer to join our dynamic team.

In this role, you will be responsible for assessing the needs of individuals within the community, developing tailored care plans, and coordinating with various services to deliver effective support.

You will play a vital role in promoting the well-being and independence of our service users.


Key Responsibilities:



  • Conduct comprehensive assessments to identify the needs and preferences of service users.

  • Develop, implement, and review personalised care plans in collaboration with service users, families, and other professionals.

  • Coordinate with healthcare providers, social services, and other agencies to ensure seamless service delivery.

  • Monitor the progress of care plans and make necessary adjustments to meet changing needs.

  • Act as an advocate for service users, ensuring their voices are heard and their rights are respected.

  • Manage a caseload of clients, maintaining accurate records and documentation.

  • Respond to crisis situations, providing immediate support and intervention when necessary.

  • Ensure compliance with safeguarding protocols and promote the safety and well-being of all service users.


Requirements:



  • Previous experience in a community care or social care role is highly desirable.

  • A relevant qualification in Social Work, Health and Social Care, or a related field.

  • Strong understanding of safeguarding practices and relevant legislation.

  • Excellent communication and interpersonal skills, with the ability to build trusting relationships.

  • Ability to work independently and manage a diverse caseload.

  • Compassionate, patient, and committed to improving the lives of vulnerable individuals.

  • A valid driving license and access to a vehicle may be required for some positions.

Pay Rate: £16.32ph
