Family Support Worker



Service care Solution are currently recruiting for a Family Support Worker in Islington to join the Disabled Children's Family Support Team



As a Family Support Worker, you will support disabled children / young people and their families through their support plans.

Their role is to act as allocated worker reviewing the plans and ensuring that they meet the assessed need.



Main Responsibilities



As a Family Support Worker, you will be responsible for:




  • Organise child in need reviews for children and young people, which are usually in the schools where the professional network can attend.




  • The review process begins with gathering information from the child, parents and professionals about the current needs and progress towards the goals in the last CIN plan.




  • Where there has been a significant change in needs or more than two years since the last assessment the FSRO would gather information to inform a reassessment led by a Deputy Team Manager.




  • FSRP's attend and contribute to Team around the Child meetings and Child in Need Reviews.

    They are responsible for completing minutes, uploading these to LCS and circulating to the attendees.



Requirements:




  • Significant experience of working with disabled children and their families

  • Relevant vocational qualification to diploma or HNC level 3

  • Enhanced DBS
