Service Care Solutions is currently recruiting for a Community Psychiatric Nurse in Chesterfield.

This is a temporary agency contract based in a community setting.

• Work as an effective professional in a multidisciplinary Community Mental Health Team.


• To prioritise and respond to urgent referrals for assessments as required and deliver high quality health and social care within the Care Programme Approach framework and to participate in duty rotas when required.


• Hold care co-ordinator responsibility for a caseload of services users and their carers.
  
  Responsibility for assessing individual care needs and the planning, implementation and evaluation of care plans.


• Work Monday to Friday, 9-5.

The pay rate for this role is £26.00Ltd an hour, paid weekly via an umbrella company.

• Approved supplier to over 400 organisations nationwide.
