Shifts: 8:00 am - 4:00 pm / 1:00 pm - 9:00 pm (Including Weekends and Bank Holidays)

Key Responsibilities:

• Provide essential housing support to individuals within the Nottingham area.


• Conduct thorough assessments of service users, identifying their needs and requirements.


• Assist with the development and implementation of personalized support plans.


• Work collaboratively with external agencies and support networks to maximize resources for service users.


• Ensure timely and accurate record-keeping, maintaining up-to-date case notes and reports.


• Engage in proactive communication with service users to promote their well-being and independence.


• Drive and implement housing-related activities, promoting a safe and supportive living environment.

Requirements:

• Valid and clean driving license (Essential).


• Enhanced DBS check (Essential).


• Availability for shift work, including weekends and bank holidays.


• Excellent communication and interpersonal skills.


• Ability to work independently and as part of a dynamic team.


• Understanding of housing-related support services.
