Community Support Worker

A full time locum contract is available in Shrewsbury for a Community Support Worker to work in a step down service for patients being discharged from hospital back to their own home.

The role sits within a very well know nationwide charitable organisation with well-established services.

Hours: Monday - Friday 9am - 5pm

Contract: Full Time - Ongoing

Pay: £13.20 ph via umbrella

Location: Shrewsbury, Shropshire

£150 for a successful application bonus!

Role:

The position sits within the Home Team supporting clients across Shrewsbury who have recently been discharged from hospital needing support with independence and implementing routines and daily structures back into their lives.

The purpose of the role is to make the discharge from hospital a seamless transition providing emotional support throughout this period.

We are looking for someone who has the skills to undertake reporting, holds knowledge of local NHS and social care services and drives so the clients can be taken out into the community if they wish.

There is no personal care duties required for this role.

If you have at least six months experience in a health and social care setting and would like to work in a vital service linked both with the NHS and leading nationwide charity, we would like to hear from you.

Candidate Requirements:

• Essential to be a driver


• Desirable to have previous health and social care experience


• Desirable to have minimum NVQ 3 level qualification relating to or health and social care


• Preferable to have a valid DBS


• Flexible towards availability
