Lincolnshire County Council are looking for a new member to join the Community Care team!

This role is a full time role working Monday to Friday 9am-5pm currently paying £12.87!

You will need to be a driver for this role as you will be out and about in the community assisting with things such as care plan assessments, supporting hospital discharge and providing care reports for people in their own homes!

This role is a very busy role where you will need to be on your toes and happy to be a busy bee!

Requirements for the role:

DBS

Must be a driver

Previous experience working with care plans and/or producing care plans

Happy to travel (miles will be paid)
