Service care Solution are currently recruiting for a Social Work Assistant with Hackney Council, to work in Adult Services.

PURPOSE OF JOB

• To provide support, information and advice and technical assistance to individuals and to develop, cost, negotiate and mediate their individual support plans as required.

• To enable individuals to take control of their support plans and resources.

• To assess people who are eligible for social care support to develop their own solutions and enhance their quality of life through promoting choice.

• To work jointly and also alone to develop support plans with people and implement these to enable people to be healthy, safe and well.

KEY ACCOUNTABILITIES

• Using person centred approaches, enable people to develop outcome focussed support plans that meet their identified needs

• To support individuals to identify potential risks and find solutions.

• To assist people to find creative solutions to meet their identified needs

• To present information about the support plan, and where relevant with the service users, to other professionals and relevant panels.

• To help people to put their support plans in place in order to meet their needs

• To work with all relevant people and organisations in support of a person to ensure their needs are met effectively, responding to timescales as appropriate

• To ensure, where necessary that all relevant information is communicated to key stakeholders/providers to ensure a person's needs are effectively met.
