Band 6 - Primary Care Liaison Nurse (RMN) - Bristol

Opportunity available for a Primary Care Liaison Nurse to work within Primary Care Team, Based in Devizes, Bristol

The team sits within the Primary Care Liaison Team based at Green Lane Hospital with the role predominately ensuring that patients in the Primary care have their mental needs more clearly assessed, defined, cared for and treated by Primary Care Health Care Professionals.

You will be able to prioritise assessments based on clinical need and risk, making evidence-based recommendations for care and treatment within and across multiple care pathways.

As part of this role you will be working in conjunction with primary acre colleagues to identify care and treatment options which enable service users to remain within their existing support and care pathways.

Some remote working may be available

This role is to commence ASAP and will be working Monday to Friday, 9am to 5pm

The hourly rate for his role is: £25.00 Per Hour
