I am currently recruiting for an adult social care assessor to work in Lambeth.

This role is initially for 3 months and applying candidates must have a current enhanced DBS to be considered for the role.

Intermediate Care is looking for tan assessment to join the team which is an integrated service with Adult Social Care services for adults and Older people.

Intermediate Care provide Urgent Response - @Home service which is an intergrated health and social care team to prevent service user's from being admitted into hospital and also provide Rehab and Reablement to service users in the community.

Candidates must be exceptional in delivery of services to service users.

Preferably Adults and Older Persons Physical Disability experience, Hospital discharge experience or other multidisciplinary team working experience.

Candidates must have the ability to work at a fast pace under pressure, be competent in carrying a caseload, working in a multi-disciplinary team, have an understanding of the Care Act, Mental Capacity Act, safeguarding and other requirements when supporting service users and their families.

A good working knowledge of MOSAIC would be an advantage.

Please note that this role requires essential face to face visits in the community, PPE is provided.
