Head of Patient Flow

Hi all, I'm currently recruiting for a head of patient flow in the NHS based over in the Shropshire area.

Hours - 37.5
Location - Shropshire
Rate - £28-£34 PAYE Per hour based off expereince

, To provide senior leadership and management support in relation to patient flow functions working in partnership with the site resilience manager
, Provide clear and visible leadership for the Command Centre, ensure the provision of accurate Sitrep and site management information as well as maintaining accurate live dashboard reporting
, To ensure the Command Centre and Patient Flow service are managed effectively working with the site resilience team, ward discharge co-ordinators, discharge lounge and transfer of care hub on a day to day basis.
, To ensure effective channels of communication exist and are maintained within and across the Trust and that there are effective working relationships and links with relevant colleagues externally where appropriate.
, To work with individual specialty areas where appropriate, to develop and implement effective practice in relation to patient flow issues in line with corporate priorities.
, To work with specialties and departments across the Trust to establish excellent relationships, robust pathways and clear mechanisms for the escalation of problems and performance tracking.

, To be responsible for the development and implementation of Trust plans, policies and protocols in relation to areas of responsibility.

Ensure these are kept up to date and that staff have appropriate training where appropriate .
, To ensure that effective leadership is provided to staff across the organisation, acting as the champion for Command Centre activity and patient flow interfacing with relevant specialities and professionals to ensure the most efficient and effective patient pathways exist.
, To work within the governance framework of the Trust ensuring that staff comply with policies and procedures
, Act as the expert for the Command Centre function and as an owner of the information contained within the live system dashboards
, To support the development of, and promote, the Trust strategy values and objectives.
, Produce accurate, high quality reports relating to areas of responsibility as required for board level committees as required by the Chief Operating Officer
, To play a major role in the improvement of the operational service delivery in relation to patient flow leading and working with clinical teams to embed best practice, deliver required efficiency and quality improvements.
, Support the provision of corporate leadership with regard to the Command Centre, acting as an expert for the function of the service and for the information contained within the system
, To support the Trust's performance management framework.

To be accountable for the performance against all key dimensions to include all access and clinical quality performance targets taking corrective action as required.
, Lead and commission review of current processes where appropriate, identifying bottlenecks and waste, and continually seeking to improve the clinical effectiveness and financial efficiency of the patient pathways.
