Early Intervention & Targeted Youth Support

I am currently recruiting for an early intervention & diversity practitioner to work in Isilington.


This role is initially for 3 months and requires candidates to have an enhanced DBS.

You must have at least an NVQ level 3 in youth justice, social work or equivalent.


Key responsibilities


1.

To hold and be responsible for Early Intervention and Diversion caseloads of children and young people including Community Resolutions, Released Under Investigation, Bail to Return and Triage.


2.

To support children and young people to positively engage with pre-screenings, assessments, plans and interventions to prevent offending, escalation and maximise support and best outcomes for the young people.


3.

Undertake purposeful home visits to inform assessments and planning with children and young people and their families/ care givers.


4.

Prepare high quality assessment of children and young people's needs, using the designated assessment framework, within the required timeframe.


5.

Attend the Early Intervention and Diversion Panel and prepare and present the required information to enable the panel to make and timely and proportionate decisions and recommendations pertaining to the needs of children and young people.


6.

Develop effective and personalised intervention plans in response to the assessed needs of children, young people and the wider community.


7.

Keep accurate and up to date records of work and interventions implemented, decisions made, actions agreed, and outcomes achieved.


8.

Deliver direct one to one and where required group work interventions to young people in line with agreed intervention framework and best practice.


9.

Broker additional support and services to meet the needs of children and young people, including making appropriate and timely referrals to other agencies and ensure the services and support are delivered in line with agreed intervention plan/s.


10.

Coordinate and attend professionals', planning and review meetings within the required timeframe/s, ensuring that all relevant professionals contribute to the intervention plan of support whilst also maximising best engagement of children and their families


11.

Be responsible for ensuring that children and young people are effectively safeguarded and protected from risk of harm or posing risk of harm to others in line with local safeguarding procedures and through robust risk management in place that is regularly reviewed, including contextual safeguarding.


12.

Ensure that children and young people and their parents are positively engage in assessments, intervention planning, reviews and exits, ensuring the views of the child are considered.


13.

Ensure that equality and diversity is integral within the case work practice and help embed this in the team culture.

Challenge disproportionality within practice in line with local action plans and strategies


14.

Ensure that the views and voices of victims are considered and that restorative justice work is central to assessments, planning and the delivery of interventions


15.

Take responsibility for own learning, training and development and participate in mid year and annual appraisals


16.

Gain expert knowledge of early intervention, prevention and diversion initiatives and incorporate elements of this into the role and embed this in practice with service users


17.

To undertake other duties commensurate to the grade of the post.
