Housing and Wellbeing Coordinator

JOB TITLE: Housing and Wellbeing Coordinator
LOCATION: Blackburn

The role of the Housing and Wellbeing Advisor is to offer customers in Later Life Services high quality housing services that support and promote independent lives.

Services will be effectively and professionally managed in line with the company policies and procedures ensuring high quality accommodation, appropriate and always stimulating activities and safety.

The post holder will actively engage with customers to promote independent living in a safe, secure, and vibrant environment always ensuring excellent communication.


  • Encourage the development of Customer Groups and Customer engagement to facilitate community and access to alternative funding streams.

    Proactively and regularly engage with customers on a range of relevant topics.

    Create an active and vibrant environment to reduce social isolation.

  • Link with local partners to facilitate community involvement.

    Develop and maintain effective and professional relationships customers and their family / supporters.

    Work with the customer to develop a plan that identifies individual needs, ensuring this is updated at least monthly and at the point of change.

  • Complete checks to ensure customer wellbeing.

    Act as facilitator / coordinator to ensure that customers receive the care and support services they require from other agencies.

  • Actively encourage the use of the communal facilities for social activities for customers, involving older people in the local community where appropriate and possible.

    Ensure a thorough assessment of needs and risks is carried out prior to allocation of property, including commitment for any care provision.


  • Local knowledge and networks.

    Knowledge and application of Group policies, procedures, and best practice.

  • Record keeping.

    Assessing and dealing with conflicting demands.

    Arrears and void monitoring.

    Support planning and risk assessing.

    Housing management.

    Representing the business.

  • Knowledge of the principles of safeguarding, support planning and risk assessing, including an understanding appropriate escalation route for high-risk matters and other relevant legislation.

    Experience of working in a service delivery, customer focused environment.

  • Demonstrated ability to manage key relationships internally and externally.

    Demonstrated understanding of the need for appropriate record keeping and associated legislative requirements e.g., GDPR.

    Ability to respond to crisis situations effectively within required legislative and policy frameworks.

Also, will be required to go through an enhanced DBS check.
