Social Work Assistant

I am currently recruiting for a social work assistant to work in Barnet.


This role is initially for a 2 month period with a potential to continue.

Applying candidates must have an enhanced DBS, and previous experience as you will be working in a fast paced D2A/ Reablement team.


Role Purpose

  • To work within a multidisciplinary environment to achieve better outcomes and quality of life for adults who need support in their community.

    Ensure that all parts of a person's individual network and community are fully explored and initiated to promote the following Social Care outcomes:

  • improved health and emotional wellbeing

  • improved quality of life

  • making a positive contribution

  • exercising choice and control

  • maintaining personal dignity

  • assisting with economic wellbeing and freedom

  • being freedom from discrimination



  • To work within the context of an enablement approach thus ensuring that everyone has the opportunity to improve their health and wellbeing through access to proportionate help and support.

    To further work with families, networks and wider community initiatives to provide an individual and person-centred approach to maintaining their independence.

    Supporting decision within individuals and families.

    To work with internal and external partners to set up appropriate ongoing support.


Key Accountabilities



  • From the initial point of contact, actively engage and communicate with people with a range of health needs, including but not limited to, physical and/or learning disabilities, mental health illnesses and challenging behaviours, and their families and support networks who may require services.

    .

    Take a proactive and whole system approach to conducting assessments and supporting and working with individuals, families and carers.

    Use or facilitate self-assessments, and be clear, supportive and knowledgeable about the various options and choices available following assessment.


  • Conduct timely, regular and frequent reviews of people's support plans, sometimes handling sensitive and highly personal situations, whilst looking at alternatives to ensure that the individual has the maximum opportunity to reach their potential and desired outcomes.

  • Support and assist people to choose appropriate options that will enhance their quality of life and maximise independence.

    Use appropriate techniques to overcome barriers to communication, such as people with sensory impairments, those with dementia or those who have suffered a stroke.

    Encourage reluctant, confused or vulnerable people, enable or negotiate with family, community or the wider voluntary sector.

  • Ensure that following assessment all appropriate community based resources have been explored with the individual, their family and/or carers.

    This could include intensive discussion and relationship building with a wide range of statutory, internal and external, and voluntary sector providers.

    The focus of this work will be to promote independence, choice and control for that individual in their community.

    The AEO will develop and maintain these strong relationships to ensure that the person has a support plan and network going forward.
