Care and Support Broker needed with West Sussex County Council

Start Date- ASAP

Location - Horsham, Worthing or Bognor Regis

Duration - Initially 3 months

Hours per week - 37 hours per week

Salary - £14 ltd an hour

Description:

• You will be the forefront of supporting the mental health customers with access to appropriate and effective care and support services


• You will deliver care planning, sourcing and purchasing functions for people requiring care and support who may be discharge from hospital, or those with longer term needs


• You may be required to provide support across all of the services, including both residential and home-based care services


• There will be a mix of office based and working from home initially

What is required?

• GCSE English and Maths, or equivalent qualification, or experience operating at this level


• NVQ Level 3 in a relevant subject, or equivalent qualification or experience


• Good understanding/experience of working in mental health and how this may impact on a customer's wellbeing, care and support needs


• Experience of monitoring data and information including financial material


• Experience of influencing and negotiating with individuals, stakeholders and service providers
