Rhondda Cynon Taff Council are seeking to recruit a Hospital Discharge Coordinator to join their team.

The coordinator will provide a single point of contact for health and social care staff at the hospital to support joined up services, providing fast effective discharge, respect for the patient's/service users preferences and the involvement of patients/service users and carers with regards to hospital discharge planning.

The key role of the coordinator will be to facilitate the transition of patients/service users from hospital to discharge, ensuring a timely and coordinated transfer between hospital, community and social care services.

This role is on an initial 3 month contract with a view to being extended on a rolling basis as the council have been granted additional funding to facilitate the position.

The pay rate for this role is £12.50 per hour and it will be a full time position for 37 hours per week.

The ideal candidate will have:
, Experience of assessing risk and how how the hospital discharge process works
, Full UK driving license
, Enhanced DBS check
, Be able to work upon own initiative and within a team
