Leaving Care Personal Advisor needed with Devon County Council based in Barnstaple

Start Date- ASAP

Contract End date- initially 3 months

Location - Barnstaple

Hours per week - 37 hours per week

Salary - £12.45 per hour

Description

• Mix of home and office based work


• Manage a caseload of Young People, 16-25


• Maintain monthly contact with young people, maintain reviews of Pathway Plans


• Promote relationships with carers and families


• Undertake interviews, observations and information gathering and to analyse, summarise and evaluate this information into an assessment of the Care leavers needs


• Ensure young people are aware of their entitlements and support them to manage their money, liaise with schools, colleagues and universities to increase educational attainments.

What is required?

• Experience of direct work with young people and their families


• Experience working with young people, families and carers, and identifying their needs


• Experience of implementing service plans on the basis of assessed needs


• Qualification in Children/Care work e.g.
  
  CQSW, NNEB, NVQ Level 3 in child or social care or youth work
