Experienced Social Worker, Transitions Team, Sutton

I am recruiting for an experienced Transitions Social Worker to work as part of the newly formed All Age Learning Disability team at Sutton.



The role will consist of working with families who have a child or children who have disabilities and ensuring that children in the borough fulfil their potential.



The primary purpose of this role is to:
, Take professional responsibility for a case load of statutory child care cases
, Ensure that every child/young person has a smart plan that ensures review and evaluation of outcomes.
, Contribute to improving the resilience and independence of Sutton's children, young people and their families
, Progress plans for children appropriately and in accordance with supervisory and management instructions and with regard to budgetary cost
, Social Workers support Families through child in need planning, short breaks, early support, looked after children visits, managing and reviewing care packages, child and family assessments, child Protection conferences and PLO work.

Hours - 36 per week Mon- Fri
£32ph

What do you need to apply?
- Previous experience in a similar role
- Support team of QSW
- HCPC / Relevant Qualification or Experience
- 2/3+ years Post qualified Experience
